To expedite processing, please review the following before submitting:
Your claim may be AUTOMATICALLY DENIED if:
- Incident occurred in non-covered zone (see map: ocr.gov.ncba/zones)
- Augment installation performed by non-approved technician
- Device warranty expired or voided by third-party service
- Claimant lacks proof of residence in covered sector for minimum 18 months
- Incident occurred during red rain event or network maintenance window
- Damage resulted from "user error" or "failure to maintain device per manufacturer specifications"
- Claimant failed to report incident within 72 hours
- Claimant has outstanding debt to NBCA-affiliated entity
- Device manufactured before 2036 (legacy hardware exemption)
- Claimant currently flagged in Tidewall Authority compliance database
- Medical documentation from non-approved provider
- Incident coincided with any declared "emergency circumstance" (HRC provision 12.4)
