To expedite processing, please review the following before submitting:

Your claim may be AUTOMATICALLY DENIED if:

  • Incident occurred in non-covered zone (see map: ocr.gov.ncba/zones)
  • Augment installation performed by non-approved technician
  • Device warranty expired or voided by third-party service
  • Claimant lacks proof of residence in covered sector for minimum 18 months
  • Incident occurred during red rain event or network maintenance window
  • Damage resulted from "user error" or "failure to maintain device per manufacturer specifications"
  • Claimant failed to report incident within 72 hours
  • Claimant has outstanding debt to NBCA-affiliated entity
  • Device manufactured before 2036 (legacy hardware exemption)
  • Claimant currently flagged in Tidewall Authority compliance database
  • Medical documentation from non-approved provider
  • Incident coincided with any declared "emergency circumstance" (HRC provision 12.4)
